Used to enter your chain of custody records for the sample. Use the Edit/Close Sample form and click on the Chain of Custody Tab: 

FIELD DESCRIPTIONS

Relinquished By: Person or client who relinquished the sample.

Received By: Person or client who received sample.

Date: Date of transfer.

Time: Time of transfer.

Notes: Comments on COC record

 

BUTTONS

	Add a COC record.  Displays the Add/Edit COC form.
	Edits the currently selected COC record.  Display the  Add/Edit COC form
	Deletes the currently selected COC record.
