Used to add and edit additional information fields to all facilities in the Hach WIMS database.

BUTTONS:

	Creates a new column in the FACILITY_I table in the current facility.  This column can be used to track extra information that the default table does not track.
	Adds a new contraint to the selected column based on another column in the database.
	Allows you to edit a constraint that has already been created.
	Allows you to remove constraints if necessary.
	Closes the additional info setup window.  (All changes are automatic, therefore no cancelation is provided)
